Instructions for doing the Evidentiary Abuse Affidavit 2.0
Before reading any further, there is one important step you must do in order to visualize what this Affidavit is all about and how you must prepare your own video testimony.

Try to include a recent photo of the abuser and show that into the video when you are providing your video testimony. Use this video to give any instructions for the care of your children/property, should any harm befall you.
Please make sure that wherever your video is being done that it is safe, and on a safe computer if using a webcam like a library or trusted friends house. Do not do this on the same computer on which your abuser has access. You can also do this on your phone, just erase it as soon as you have uploaded the file. Other places to consider would be a church/place of worship, domestic violence shelter,
If you keep a copy of this document in a safety deposit box be sure someone you trust has an extra key to the box and access on a signature card with the bank.
This is a web based app, please write down/remember your user name and password and store it somewhere safe. Also, the EAA will ask for an email address, please make sure it is NOT one the abuser can access such as home email. You may want to create a new email just for this purpose, use a work email or a trusted friend/family member) When filling out the information on the app, please press save and return after each page so if you aren’t able to finish it at one time your information will be saved.

Safari will keep your browsing history private for all tabs in this window. After you close this window, Safari won’t remember the pages you visited, your search history, or your AutoFill information.
Additional Useful Information to add to your EAA
Phone Threats
Make copies of any and all phone threats received on your cell phone or home phone. If you have a smart phone there are several options to sending or saving messages. If you look at the voicemail there is a upload icon on the top right. There are several options in which to either send or save the messages. You can message it to someone you trust, if you have an IPhone you can airdrop to another phone or computer as a file and upload it in the place provided in the app. Also, on a separate sheet of paper write out the name of your cell phone carrier with the cell phone telephone number and password access code to your cell phone. (This will direct law enforcement should you go missing)
Email Threats/Correspondence
All threatening emails or letter received in the mail, including cards with disturbing words are to be copied, included and kept with this new document. Remember e-mails show the date and time sent when printed.
Collection of DNA Samples
In the event that something should happen to you, DNA evidence can be crucial. If you don't have a good picture of your abuser, you should have something with his DNA in a secure place. Collect hair samples from a comb or brush that he has used, a razor, old dental floss, a hat or ball cap, cigarette butts or anything which bodily fluids may have come in contact. Place any or all of these samples into a sealed, plastic bag and label with the name and date.
You should also do the same with samples of your own DNA. In the event that you may go missing this sample could mean law enforcement agencies can move much more swiftly.
